The sooner the better, but it cannot be filed until there is an actual "Case or controversy", that is some actual person has to be turned down for insurance coverage of GID and the lawsuit must be filed in that person's name.

Not necessarily. Estrogen is prescribed to non-trans women. There is no good reason why the exact same diagnoses used for such prescription should not be applied equally to transwomen.
All it takes is willingness and the absence of the GID diagnosis. It does not take any changes to medical manuals such as the ICD.

Proven effective treatments will not go away merely because the rationale used to justify trying those treatments is debunked.

What we need is for GID to be put into a physical medicine catalog of conditions. It is not a disorder, but it IS a birth condition.

The birth conditions do not "need to be put" into physical medicine catalog of conditions -

- for reason that they are already present in the International Classification of Diseases (ICD) published by the United Nations Organization (http://www.un.int) and therefore no longer need to be added.

See for example Q50-Q56 in the ICD-10
http://apps.who.int/classifications/apps/icd/icd10online/index.htm?navi.htm+q00
equivalently http://snipurl.com/s1323
within http://apps.who.int/classifications/apps/icd/icd10online/

.
What needs to happen, is for GID to be removed from the DSM and ICD so that the correct diagnoses will be used instead. And thus transwomen will receive the same treatments, and on the same basis, as non-trans women who have the same needs.

Equality in treatment is really not to much to ask for, it is past time to reject those who tell us not to be silly because we are inherently unequal in privilege.
